You will be disenrolled from Healthfirst Medicare Plan in the following circumstances:
If you enroll into another plan, Medicare will notify us and we will update your records and send you confirmation of your disenrollment. If you did not intend to disenroll from Healthfirst Medicare Plan, you have the right and responsibility to ask us to cancel your disenrollment. But you must notify us before the last day of the month you are enrolled in our plan.
If you live outside of the service area for more than 6 months in a row, we will send you a letter asking for confirmation of your address before we send you notice of your disenrollment. Medicare requires us to disenroll any member who lives outside of our service area for more than six months at one time.
If you move out of the service area permanently, you have a right to enroll in a Medicare Advantage or Prescription Drug Plan in your new community but this option has a time limit. By enrolling in a new plan, you will be disenrolled from Healthfirst Medicare Plan. If you do not enroll into another plan during the limited time allowed by your move, you will have to wait until the Annual Enrollment Period (AEP) to choose a new plan or to disenroll from our plan or you will be disenrolled from our plan after six months of living outside our service area.
