Beginning May 4, 2012, the following Soundview Health Centers will no longer be in the Healthfirst provider network. If you are getting care from one of the listed provider locations below, you will need to choose another Primary Care Provider (PCP) or Specialist. You may also contact our Member Services Department at 1-888-260-1010, 7 days week from 8:00 am through 8:00 pm. TTY users should call 1-888-542-3821 for any questions regarding this change or how it may affect your care and to arrange for care with another participating hospital or doctor.
