How to Renew Your Health Insurance
Every year, you will be asked to submit a health insurance renewal form that will determine if you are still qualified for the Healthy NY program. Your health plan will notify you when this form is due and will provide you with all necessary forms. You must submit this form every year to keep your Healthy NY insurance.

Don’t lose your health insurance!
For some Healthfirst plans, you will need to make a monthly payment (or “premium”) to stay insured. It’s important to make all payments on time. You can make payments online, by phone, or at any Healthfirst office in your neighborhood.
