Healthfirst Launches CompleteCare, a New Comprehensive Medicare/Medicaid Care Option

NEW YORK, NY, June 1, 2012 – Healthfirst is announcing the launch of CompleteCare, a fully integrated Medicare/Medicaid long-term care (LTC) product.
CompleteCare is available to individuals who are eligible for both Medicare and Medicaid who need long-term care services in the community. It brings together services from both programs under one product, offering both managed medical and long term care benefits to an especially frail and vulnerable segment of the population.
Each CompleteCare member will have a three-person care management team – a registered nurse (RN), social worker, and support coordinator – who will coordinate the member’s medical and long-term care needs through a single point of contact. This will offer a seamless, integrated approach that will aim for better outcomes and high member satisfaction, and allow members to live safely in their homes – instead of a nursing home -- for as long as possible.
