Medicaid Managed Care

Did you know that some Medicaid recipients can now choose a managed care plan?
In New York City, choosing a Medicaid managed care health plan is voluntary until you receive an official notice from New York Medicaid CHOICE informing you to enroll in a health plan. At that time, you will have several managed care plans in your neighborhood to choose from. If you are a resident of New York City, Nassau or Suffolk Counties, choosing a Medicaid managed care health plan is mandatory. On your recertification date for Medicaid, you will be required to enroll in a managed care plan. If you do not choose a Medicaid managed care plan, one may be chosen for you.

  • Preventive

To be eligible you must receive assistance from:
  • Temporary Assistance to Needy Families
  • Medicaid ONLY (TANF & MA-ADC)
  • Safety Net Assistance (SNA)
  • Supplemental Security Income (SSI)
  • Reside in the following Healthfirst service areas:
    Manhattan, Bronx, Brooklyn, Queens, Richmond/Staten Island, Nassau and Suffolk Counties.

If you meet the above requirements you may be eligible for Medicaid Managed Care Plan. Click here to send us your information so a Healthfirst Representative can assist you in enrolling in the plan. If you think you are not eligible for Medicaid you may be eligible for Child Health Plus.
