To meet your needs, Healthfirst is proud to offer Family Health Plus. Designed by New York State, this program helps certain adults get needed healthcare.
Working adults must meet income guidelines slightly higher than what is allowed for Medicaid. Healthfirst has joined Family Health Plus to provide a program that gives individuals their healthcare coverage through the Healthfirst network of affiliated physicians.

Adults are eligible if they are:
- U.S. Citizen, Permanent resident, or fall within one of the many immigration statuses
- Ages 19 to 64
- Not eligible or enrolled in Medicaid
- Reside in the following Healthfirst service areas: Manhattan, Bronx, Brooklyn, Queens, Richmond/Staten Island, Nassau and Suffolk Counties

If you think you are not eligible for Family Health Plus you may be eligible for Medicaid Managed Care Plan. |
