2014 Healthfirst Medicare Plans

Medicare eligible individuals deserve great health care and special benefits.
How can you see a doctor without worrying about the cost? Find out if you or your loved ones are eligible for a Medicare Advantage health plan offered by Healthfirst.

We can help you pick from several different Medicare Advantage health plans so you can get the coverage you need. You can choose a plan with or without prescription drug coverage. If you qualify for Medicaid, we also have Medicare Advantage plans that will work with Medicaid to give you the extra coverage you need.

Medicare Life Improvement Plan

Healthfirst Life Improvement Plan (HMO SNP)

This plan is for:

Medicare beneficiaries who are eligible for both Medicare and some level of assistance through New York State Medicaid (“dual eligible”).


Medicare Maximum Plan

Healthfirst Maximum Plan (HMO SNP)

This plan is for:

Medicare beneficiaries who are eligible for both Medicare and full Medicaid benefits through
New York State.


Healthfirst CompleteCare (HMO SNP)

This plan is for:

Medicare beneficiaries who are eligible for both Medicare and Medicaid benefits with added long-term care services.


Healthfirst AssuredCare (HMO SNP)

This plan is for:

Medicare beneficiaries who meet specific enrollment criteria such as needing nursing home care.


Medicare Increased Benefits Plan

Healthfirst Increased Benefits Plan (HMO)

This plan is for:

Medicare beneficiaries who also get extra financial help from the government, for health needs such as prescription drugs (Part D), or in the form of Medicare Savings Programs (MSP).
 


Medicare 65 Plus Plan

Healthfirst 65 Plus Plan (HMO)

This plan is for:

Medicare beneficiaries who do not qualify for extra financial help.

  • Not qualified for Low Income Subsidy (LIS) for Part D
  • Not qualified for Medicare Savings Programs (MSP)

Medicare Coordinated Benefits Plan

Healthfirst Coordinated Benefits Plan (HMO)

This plan is for:

Medicare beneficiaries who need a Medicare Advantage program, but get prescription drug coverage from another source or do not need prescription drug coverage.

 

This plan does not include prescription drug coverage. To make sure your medicines and health supplies are covered, you should receive prescription drug coverage from sources other than Medicare Part D (e.g., Veterans’ Administration).
