2013 Maximum Plan (HMO SNP) - Plan Summary

The Healthfirst Maximum Plan (HMO SNP) is a Medicaid Advantage Plan designed specifically for “fully dual-eligible beneficiaries” – people who have Medicare and receive full Medicaid Benefits through New York State. As a Medicaid Advantage Plan, the Maximum Plan provides seamless coordination of both Medicare and Medicaid benefits for our members.

Don’t lose your health insurance! Make sure you know what you need to do to stay insured.
For some Healthfirst plans, you will need to make a monthly payment (or “premium”) to stay insured. It’s important to make all payments on time. You can make payments online, by phone, or at any Healthfirst office in your neighborhood.
