2013 Coordinated Benefits Plan (HMO) - Plan Summary
The Healthfirst Coordinated Benefits Plan (HMO) is a Medicare Advantage Only plan. This means the Coordinated Benefits Plan does not include prescription drug coverage (also known as “Medicare Part D”).
The Coordinated Benefits Plan is a good choice for Medicare beneficiaries who do not need prescription drug coverage, or who receive prescription drug coverage from sources other than Medicare Part D (e.g., Veterans’ Administration).
Don’t lose your health insurance! Make sure you know what you need to do to stay insured.
For some Healthfirst plans, you will need to make a monthly payment (or “premium”) to stay insured. It’s important to make all payments on time. You can make payments online, by phone, or at any Healthfirst office in your neighborhood.
