The Healthfirst AssuredCare (HMO SNP) is a Special Needs Plan (SNP) designed specifically for Medicare beneficiaries who are eligible for Medicare and also meet specific enrollment criteria. If you reside in a nursing home or require the same level of care as someone in a nursing home, you may be eligible to join this plan. Through our Model of Care, this Special Needs Plan provides Medicare benefits and Skilled Nursing Facility (SNF) care.
